Manager Info Letter
TO: Team Managers
FROM: Pahl Zinn RE: Registration Procedure DATE: September, 2009 _______________________________________________________________________ In order for your team to participate in any scrimmage or league game, your team must be officially registered with the District #3 MAHA Registrar. To do that, you must provide the following documents to me at least 1 week prior to your first scrimmage/game: Players must provide: 1.Each player’s birth certificate. Only birth certificates can be accepted; not hospital records, baptismal certificates, etc. Make a copy of the certificates for your own file – you will need them if you go to districts. If you have a player who was born outside the U.S. and has dual citizenship they must provide proof. If the player is not a citizen of the U.S. they must submit a copy of their Alien Registration Card. 2.Consent to Treat form 3.USA Hockey registration confirmation email Go to USAHockey.org and choose Membership on the navigation bar across the top. The fee is $35. 4. Participant Code of Conduct 5.S.T.A.R. Parent Code of Conduct. This must be signed by both parents. 6.GPHA Membership Agreement Coaches must provide: 1.USA Hockey registration confirmation email 2.MAHA Background Screening confirmation email Go to MAHA.org and choose Forms & Info across the top, then choose Background Check on the left-hand side. MAHA has now included the Coaches Ethics Agreement within the Background check so that is no longer necessary to complete separately. 3.Participant Code of Conduct 4.Consent to Treat 5.Copy of both sides of coaches 6.Student Coaches Information Form if coach is under the age of 18. Managers must provide: 1.Waiver of Liability, Release Assumption of Risk and Indemnity Agreement. Managers are not required to register with USA Hockey, but the waiver of liability which is contained within the USA Hockey registration is still required. 2.MAHA Background Screening confirmation email 3.Participant Code of Conduct 4.Consent to Treat Please make a copy of all forms for your own file. You will need them if you go to districts. Put all of the same forms in one group in alphabetical order and clip together. I should have 8 groups of forms (you can include the manager’s Waiver of Liability with the USA Hockey forms). For my own sanity, I cannot accept incomplete packets. If you are having a problem completing the packet call me immediately. Once I receive your packet in the format I explained above, the following has to happen: 1.I will then enter the data and print out the two USA Hockey Roster Forms. 2.I will then e-mail the roster form to the manager who will need to get the player, coaches and manager signatures and then return them to me Warning – Check your official USA roster for accuracy and completeness. Players must sign their name exactly as it appears on the official USA Hockey roster -- no shortened forms, middle names or nicknames will be accepted. Notify me immediately if any additions or corrections are required; do not have the inaccurate form signed. 3.I will provide the signed roster form to the District Registrar. 4.When the District 3 Registrar has signed your official USA Hockey roster, then your team will be officially registered. I will call you to arrange for you to pick up your completed packet from me. 5.Warning –No one is permitted on the ice, except rostered players and coaches.
