Team Rostering Process & Examples
The following information will be needed in order to roster your player for a team: 1) Copy of player's birth certificate 2) Consent to Treat (download) 3) USA Hockey registration (copy of email confirmation) Go to USAHockey.org and choose Membership on the navigation bar across 4) Participant Code of Conduct (download)the top. I think the fee is $35. Please forward your email to your manager so they have it for the rostering package. 5) Parent Code of Conduct (download) 6) GPHA Membership Agreement (download) Coaches: 1) Background Screening (copy of email confirmation) Go to MAHA.org and choose Forms & Info across the top, then choose 2) Participant Code of Conduct (download)Background Check on the left-hand side. According to the MAHA website, they have included the Coaches Ethics Agreement within the Background check so that is no longer necessary to complete separately. Please forward your email confirmation to your manager for their rostering package 3) Consent to Treat (download) 4) Copy of both sides of coaches card Manager's Examples: Contact List Sample Budget MAHA Game Log 2009 District Credential Requirements All standard forms are linked to this page.
